A Medically Underserved Area (MUA) by state is most clearly compared by counting designations: HRSA currently records 4,155 MUA designations across 59 states and territories. Each designation applies to a whole county, a group of counties, a group of county subdivisions, or a group of urban census tracts, so a state's designation count reflects how many distinct areas HRSA has certified as underserved — not how severely underserved any one of them is. That second question, severity, is answered separately below.

Key findings
California leads with 214 designated MUAs, and the next two states, Texas (212) and Illinois (176), keep the top of the table concentrated: those three states alone hold about 14% of every MUA designation nationwide. Designation count tracks population and geographic size as much as underlying need — large, populous states simply contain more counties and census tracts that can each qualify.
Designation count and designation severity are different questions, and they do not track together. California tops the count leaderboard but does not appear in the severity ranking below — it has the most designated areas, not the most severely underserved ones on average. Reading the two rankings together, rather than either alone, is what keeps this comparison honest: a state with many designations can still have a moderate average severity, and a state with far fewer designations can have a much deeper average shortage where they do occur.

Why count and severity are both necessary
HRSA's Index of Medical Underservice (IMU) combines four measures — the ratio of primary care providers to population, the infant mortality rate, the share of the population below the federal poverty level, and the share of the population age 65 or older — into a single composite score. An area scoring 62.0 or below qualifies for MUA designation, and lower scores indicate greater underservice. Because the IMU is scored per designated area rather than per state, a simple state average only means something once you also know how many areas contributed to it: an average built from five designations is a much noisier signal than one built from a hundred, which is why this page's severity ranking is limited to states with at least five designations.
The count ranking above answers "where is MUA designation most widespread"; the severity ranking answers "where does the average designated area fall furthest below the underservice threshold." Both are legitimate lenses, and federal placement and funding decisions can use either depending on whether the goal is broad coverage or targeting the deepest need.
Limitations
Designation practice varies administratively by state and territory, so two places with similar underlying access can record different counts depending on how aggressively they pursue and renew MUA status. The severity ranking above excludes 124 records fleet-wide (concentrated in Puerto Rico) carrying an exact IMU score of 0.0 — implausible as a genuine composite score against the observed ~24–75 range for scored designations, and treated here as missing rather than a real value; a jurisdiction whose designations are dominated by these records will not appear in the severity table even if it ranks highly by count. MUA status reflects a structural access measure at the time of designation, not a real-time count of unmet appointments, and does not measure the quality of care residents who do reach a clinician receive. Figures reflect designated MUAs/MUPs only and come from the portal's source-vintaged HRSA snapshot.
